Appointments
To schedule an appointment please contact us by phone at 734-475-3221 during office hours. Our office hours are 8:00 a.m. - 12:00 p.m. and 1:00 p.m. – 5:00 p.m., Monday through Thursday, and 8:00 a.m. - 2:00 p.m. on Friday.

New Patients

Forms/Paperwork:
Please arrive 15 minutes prior to your scheduled appointment time. Required forms will be provided to you upon your arrival. Be sure to bring the following with you:
• Insurance Card(s)
• Photo Identification
• List of current medications
• Payment method for your required co-pay
• If required by your insurance company, the proper referral from your Primary Care Physician. If you are unsure whether or not you need a referral, please contact your insurance company.
Insurance
Please bring your insurance card with you for your visit. Without this verification of insurance, full payment for your visit will be due upon checkout.

Check-In
Please note that we ask you to arrive at our office 5 minutes (15 minutes for new patients) prior to your appointment time with the physician or physician assistant, so that we may complete the necessary administrative and clinical duties which accompany every visit.
This added time allows us to provide you with the level of service that we strive to achieve for each and every patient.
Location and Parking
Our offices are located on the third floor (Suite 308) of the Professional Building on the campus of Chelsea Community Hospital. Click here for address, directions and map information.
Free parking is available on site at the Professional Building.
Prescription Refills
Please have your pharmacy electronically submit your prescription refill requests to our office.
Cancellation and Late Policy
If you should need to cancel your appointment, kindly provide at least 24-hour notice.

Please note, in some cases our physicians may not have another open appointment for several weeks. Appointments canceled with 24 hour or greater notice will receive priority in rescheduling.

In consideration of our other patients, we have enacted a late arrival policy. Please understand that if you arrive more than 15 minutes late for your check-in time, we may have to reschedule your appointment.
We know that your time is valuable and we make every effort to see you in a timely manner. However, the quality of your healthcare and that of other patients is our highest priority. Therefore, there may be times when you or another patient simply require more time than was anticipated. We appreciate your patience and understanding, and apologize for any unforeseen delays in your visit.
Billing and Payments
We will submit your claim to your insurance carrier on your behalf. Submission of claims is not a guarantee of payment. Please refer to your policy for coverage and benefits.

Co-pays required by your insurance company are due at the time of service. Please expect to pay these co-pays at the end of your visit.

Submitting a claim does not guarantee payment by your insurance carrier and any balance unpaid by the carrier will be your responsibility.

If you have concerns about a procedure being covered, please contact us or your provider to clarify what will be covered as part of your policy.
Please provide a current insurance card at check-in for your appointment.

Please note, cosmetic procedures are not covered by insurance plans. Be prepared to pay for these procedures at the time of your appointment.
